This participatory action research is aimed to developed model for participation of community network advanced cancer patients received palliative care in the community. The participants included 12 dyads of community network in palliative care (CNPC),  a nurse at sub-district health promoting hospital, 8 village health volunteers (VHVs), the Deputy Chief Executive of sub-district administrative organization (SAO), a local leader, and a sub-district leader. Data collection used interview schedules, in-depth-interview guide, focus group discussion guidelines, and observation checklists. Data analyses used descriptive to percentage, mean, standard deviation, and analytical statistics for quantitative data, and content analysis for qualitative data. The study comprised three phases. Phase I prepare phase, situation analysis, the results showed that CNPCs had low level knowledge, perception skills and satisfaction with palliative care. Phase II acting phase, model development and planning, model implementation and assessment, the results showed that the integrative was training skill knowledge of holistic palliative care, social support of CNPCs and follow-up home visits of ACPs , the development of capacity CNPCs to participated in ACPs, the integrated home visits by CNPCs, including Buddhist monks, community leaders (CLs), sub-district administrative organization presidents, community network volunteers (CNVs), student volunteers (SVs), and public health care teams (PHCTs). Phase III summary evaluate follow-up assessment and model remodification, the results showed that 1) the participatory model developed was called “ The Community Network Participation Model in Palliative Care of Advanced Cancer Patients”(CNP_MPCACP).  It was composed of 1) CNPCs for ACPs, 2) the roles of the CNPCs in providing social support to the ACPs and 3) the home visits and the home environmental modification. This eight-week model was applied to all. CNPCs agreed upon its potential for implementation at a high level. The satisfaction of the CNPCs changed. The CNPCs’ knowledge, perceptions skill of palliative care were improved significantly at post-test and at follow-up (p<0.05)
